外傷處理原則-傷口處理

簡介

外傷是急診常見的問題，其順位放在secondary survey，要先把primary survey做完才處理。

• Primary survey：ABCDE
  A(airway)B(breathing) C(circulation) D(disability) E(exposure)
  (Disability: 看病人的意識狀態、coma scale)

• Secondary survey：病史、從頭到腳檢視、重點理學檢查等

傷口處理原則

(1) 先將傷口用水沖乾淨→做好debridement

(2) 清理掉foreign body→在傷口周邊或更深處，容易會miss掉

(3) 傷口吻合：傷口兩端分開<3mm者，可以不用縫

• 頸部以上的傷口：給予頭部外傷注意事項衛教
• 毛髮要剃掉，消毒才完整 (但不要把眉毛剃掉！很難長)
• 縫合不要留deadspace、縫的時候不要tie太緊
• 縫完後，傷口加壓15分鐘

(4) 打破傷風疫苗(Tentanus taxoid)

• 劑量：小孩和成人都是0.5cc
• indication
  • 沒打過3次，要補一針(通常6歲前不需打，小時候有打疫苗了)
  • 乾淨傷口：10年內沒打，補打一針 (乾淨傷口→刀割深度<1cm)
    髒的傷口：5年內沒打，補打一針
• 懷孕女性也可以施打 (Ref: 2011 ACIP的建議)
• risk：小心疫苗過敏的狀況

拆線

• 臉部：blood supply佳，約3-5天拆線 (用simple suture)

• 軀幹需要7-10天，四肢需要10-14天，近關節可能要14天以上
